York Rite- Pennsylvania

I can't find online a petition for the York Rite in Pa. I was given one by a member of my lodge that I filled out and I was going to give it back to him tonight but I can't find where I placed it. I was hoping to find one online for PA but haven't yet. If anyone can help I would be forever thankful.

Thanks Bro Gary, that's what I am going to use. From what I was told Pa isn't a member of the General Grand Chapter/Council/Commandery and is their own body which is why I am afraid that petition might not work since I know the one I filled out 1st didn't look exactly like that.

I'm sure they will give you another one. It was the only one I could find that may have been suitable. I know of one for Florida, but that one would most likely not be what you need.
